To assist with contact tracing
Please use this form to let us know if your child tests positive for covid-19 up to and including 18th February.
ONLY SUBMIT THIS FORM IF
- your child currently attends school (not home learning)
- their symptoms started on or before Sunday 14th February
- and they were in school on either of the two days before their symptoms started (or before they were tested if they were asymptomatic).
If symptoms developed on Monday 15th or later, or if the child was not in school during the 48 hours before their symptoms developed, then people in school will not be identified as contacts and will not be affected.
If your child develops symptoms after Sunday 14th and subsequently tests positive, you do not need to let us know during the school holidays. Please contact school at the beginning of next term to let us know when their period of isolation is due to end.
If your HJS children are fine, but a family member develops symptoms, you do not need to contact us immediately. Let us know in the usual way at the start of next term if your child will be isolating and unable to attend.
